the part where there are smoke like things coming out from the head and hands. . .

Those are strands of hair, if thats what your meaning. Just painted those in. And stuff around the hands and what not are just blood trails. Just picked a light red color, set it to color burn, low opacity, selected the brush to pen pressure and started paintin.
